This agreement is between you, the CUG (Closed User Group) user, and the Foundation for Head and Neck Oncology (FHNO). Acceptance occurs upon your use of the portal for any transactions.

• Acceptance of Agreement: You agree to the terms and conditions outlined in this Agreement with respect to our site. This Agreement constitutes the entire and only agreement between you and FHNO, superseding all prior agreements regarding this site. This Agreement may be amended at any time without specific notice to you. The latest Agreement will be posted on the site, and you should review this Agreement prior to using the site.

• Service Delivery: FHNO reserves the right to deliver services as outlined on this portal. Should you have any complaints or disputes with the services provided, you are encouraged to contact our office directly for resolution.

• Privacy Policy: We collect personal information, such as contact details and transaction history, during your use of our portal. FHNO reserves the right to use this information for communication purposes through email, mobile, or postal mail.

• Terms of Service: FHNO reserves the right, at its discretion, to refuse service to anyone.

• Modifications to Terms and Conditions: These terms and conditions may be modified without prior notice.

Membership Registration & Eligibility:

Required Documents:

  • Highest Eligibility Proof (Degree certificate/Fellowship proof) (jpg/PDF file format, 100-200 kb file size)
  • Valid Photo ID (Aadhar, Voter ID, Passport) (jpg/PDF file format, 100-200 kb file size)
  • Council Registration (if applicable) (jpg/PDF file format, 100-200 kb file size)
  • CV (Doc/PDF file format, 100-200 kb file size)
  • 2 Letters of Recommendation (jpg/PDF file format, 100-200 kb file size)

Submission Timeline:

  • Submit documents within 3 days of successful payment.
  • Incomplete submissions may delay or cancel the registration.

Declaration of Authenticity:

  • Tick the checkbox to confirm document authenticity.

Membership Status:

  • Provisional membership granted upon verification.
  • Confirmed membership at next General Body Meeting (GBM).
